Your Initial Visit

The initial visit will be mostly evaluation and will be aimed at identifying the cause of your problem. We will discuss your reasons for coming to physical therapy and your medical history. You will receive postural analysis and tests of flexibility and strength. Hands on evaluation will include palpation of muscles, tendons, joints, connective tissue (fascia) and the craniosacral system. We will begin treatment as well.

 

Please bring the following:

✓ Lightweight shorts (like running shorts).

✓ For women a tank top or bra top. Underwear is also fine.

✓ List of any medications you may be taking.

✓ Written copies of any test results such as x-rays or MRI’s.

Note: If you are taking pain medication, there is no need to stop taking it for the evaluation.

Insurance Coverage

Finn Physical Therapy is out of network with all insurance companies. Payment is expected at the time of treatment.

If you have out of network coverage, your insurance company will reimburse you for a percentage of the session fee.

I will provide you with a bill with all the necessary information to submit to your insurance company.

Here are some questions to ask your insurance carrier:

  1. Do I have out of network physical therapy benefits?

  2. What percentage of physical therapy will be covered?

  3. Do I need pre-authorization for treatment?

  4. What is my out of network deductible?

  5. Do I have a maximum number of visits per calendar year or per condition?

  6. Do I need a referral from my primary care physician?

  7. Do I need a referral in order to be reimbursed for treatment?

  8. How do I submit my claim for physical therapy?

Doctor Referral / Prescription


You may receive treatment for 10 sessions or 30 days without a prescription, whichever comes first. After this, you will need a prescription from your primary care doctor, any specialist or your dentist.

However, some insurance carriers require that you have a prescription in order to be reimbursed. Please check with your insurance company if you plan to submit claims.

 Cancellation Policy


Sessions are 1 hour long.  I accept cash, personal check, Chase quick pay (Zelle), and Venmo for payment. Credit cards are accepted with a $5 fee.


Please provide at least 24 hours notice if you need to cancel your appointment. The full fee will be charged for late cancellations and missed appointments. You may reschedule your appointment on the same day without a charge if an opening is available.
