Finn Physical Therapy is located at 611 Broadway, Suite 801, NY, NY 10012.
It is at the corner of Broadway and Houston Street, in the Crate and Barrel building.
Nearby trains are the N and R at Prince Street, the 6 at Bleeker Street, and the F,D, B and M at Broadway Lafayette.
The Broadway and Houston Street buses stop close by.
What are the business hours?
Do I need a prescription for physical therapy from a doctor?
Due to recent legislative changes in New York state you may have 10 visits or 30 days of physical therapy without a prescription, whichever comes first. After that you will need a prescritpion from a primary care physician, a specialist, or a dentist.
Do I need a referral from my primary care physician?
Some insurance companies require a referral , as well as a prescription from a doctor. Please check with your insurance company for their requirements.
What should I bring on my first visit?
Please bring lightweight shorts (like running shorts), and for women a tank top or a bra top. Underwear is also fine. As I work with a holistic approach, I evaluate and treat the whole body. It is better if I can see your whole body alignment. You may change at the office.
Please bring a list of any medications you may be taking, and written copies of any test results such as x-rays or MRI’s.
What time should I arrive for my first visit?
Arrive at your appointed time. Please knock at the door. If early you may wait inside the office.
How long is the appointment?
All sessions, including the first one, are an hour long.
How do I pay?
Payments can be made by personal check or cash.
